You mean in general, or all tied up and forced like in the video? If the former, if you are capable of rubbing one out in the shower (a lot of guys here seem incapable for some reason), re-lube and rub whatever it is you're using on the head. You'll see. Or feel, rather. I wouldn't describe it as pain, because it doesn't hurt, per se, but whatever it is, it's unbearable without some very good mind over matter concentration and determination.
