I find doing them in a muslim prayer-like position isolates the relevant muscles best (not just for the pun, I promise), and I generally just do one long hold for the 20 minutes, or until I cant hold any longer; I then start again and finish the 20 at least. This isn't so bad as I can also read/watch tv in this position).
About a month later and my cock is getting back on form and looking pretty good, as is the ladies front.
