Clean, well-cut, flattering underwear is important. It's a good indicator that the guy is well-groomed and that I'm going to find something exciting inside said undies.

Demeanour, general dress sense, and "sparkle" are better initial indicators. Unless I'm meeting him for the first time and he happens to be starkers.
