You know when you lift your hand from a lighted surface and the shadow grows enormously. The penis is elevated from the X-ray plate or table he’s laying on. So the penis is magnified.

It depends on the technique used by the X-ray tech. A hotter technique could burn out the foreskin in an X-ray image. But from the picture posted, it was more of a soft tissue technique so I would say that he is circumcised.

Actually in Xray, Radiologists have to view images in anatomical position. So his penis is actually pointing to his Left. But we see it pointing to our right. Think of it as looking at a person standing in front of you.

you were a one piece bedclothes and your naked below it, the xray is taken when wearing that so your not nude when in xray room. having the one piece makes it easier for hospital staff to get access to areas and examine you. they are not fun to sleep in but are pratical from hospital point of view.
