I don't use it often now but I did use the Aneros with regularity for a while. My experience is that you don't need to douche, but put down a towel anyway. It does take practice. The first several times I felt nothing.
The key thing is to find out what level of tension to maintain with your muscles down there. First, become really familiar with a kegel. (Don't confuse clenching your butt cheeks with a kegel.) Practice varying degrees of contraction. See if you can assign a percentage contraction to what you are doing form 0 to 100% contracted. Lube up (use a lot!) with a really slippery lube and reapply several times. I like the gel lube because it seems to last longer for me. Insert and play with several levels of contraction. I find I had the best results lying on my back with my knees up.
What I found, for me, was I finally got the results I wanted by oscillating between about 30% to 60 % of full kegel contraction. Make one complete oscillation about every one to two seconds. You should feel the probe sliding back and forth inside very smoothly. Keep doing it for 5 to 10 minutes if you can. Take a break if you are getting tired or frustrated. You have to be relaxed. It's not like a regular orgasm where you can "get on it" and make it happen. It kind of has to come to you. It's also like training a new neuronal pathway in your body. Try not to play with your cock much because you will want to go into your usual pattern and stop the new response that you are looking for from arriving. You may get hard, but most likely not. Plan on laying out a half hour session. Do at least 6 of these sessions to really get it. The different kind of orgasm first started with a tremulous feeling in my thighs and hips. The key is to let it happen. Gradually the tremors started to slowly build and become more strong and involve more of my body. Then it becomes sort of like relaxedly riding a wave, all the while you are working your internal muscles oscillating the device. It is not an easy thing to learn initially. But neither is riding a bike or skiing. But it is a very cool experience when you get it - worth it. You may or may not want to do it regularly but it is a unique experience.
I hope that helps.
