I would start with probiotics , diet, cardio and some weight lifting. Maybe some testosterone boosting supplements if you don’t see some improvements in a few months. Wouldn’t go on trt.

There is a link between the gut and brain. It’s helpful for everything including erections. Research it.

I tried a bunch of different probiotics/foods and a few times had a viagra like response while overseas with some liquid probiotics.
You could also research maca, l arginine , pine bark and other boosters. But don’t skip the cardio and other stuff.

I exercise and cycle supplements( occasionally ) and probiotics/probiotic rich foods . Lots of swollen night time erections and high sex drive. Slowly getting thicker but satisfied with my 8x7. I was a long time pumper so that may also play a role .
Testosterone levels were a little low in 2020 at 400. Now they are 550-600 and I don’t need them any higher. Early 40s.
