There are many way to rip a condom. Too tight, too lose, passed expiration date, wrong type of lube (oil based lube tear them apart as if they were biodegradable shopping bags tat barely last from the counter to the car)

If a condom is too tight can it break? It has only happened to me once but I think it might be because the condom was old or something. But it's something that always has me worried that it could happen.

Yes. If it's too tight, not tight enough, old and dried out (as you say it might have been), exposed to non-water based lubricant, etc., it can tear. This is why it's important to learn the correct method of applying a condom and other proper practices (as well as proper size in certain cases).

Ok, this is important; DO NOT DOUBLE BAG IT. The friction between the two condoms will, more often than not, lead to tearing, and even if it doesn't, it will not result in bolstered protection. It's a real lose-lose (or lose-lose-lose if you consider it will likely make the experience feel worse, too).

They have all sizes and even non latex which is extremely important.

Non-latex is nice—feels better, no rubbery smell—but if you're worried about breakage, watch the fuck out

I used polyurethane a lot in high school (gf had a latex allergy) and they tore very easily. Led to...problems

Yes. I broke a few when I was around 20 with my ex girlfriend. We had a scare but thankfully she didn’t get pregnant. Each time I thought “wow, I can barely feel this thing on” and pulled it out and it was just the ring and torn rubber hanging off. It shredded. Happened 3 times before I just started pulling out instead.
In hindsight I should have gotten larger ones. Young and dumb and full of...you get the idea.
