The underwear itself doesn't come with a fake penis extension, it's only underwear. It also doesn't look like he has his penis stuffed above any potential fake extension, but rather just a slight fat pad from his weight. This is also before he was massive YouTuber/streamer(decently sized at the time) and he had done other "risque" things for laughs before and after this, so it's probably just his penis.
