Pretty simple: if it's painful, it's too tight. You can safely wear a properly sized one indefinitely.

Don't use the steel ones if you're going to get hard-ons or get any bigger, use rubber or leather, I prefer snap-ons to change the snugness back and forth. Leather Snap Ons are good but dont care for rubber as things tend to get pinched. Use steel doughnut ring to give weight when not planning a stiffy.
