I remember a flight to London once with a 2 year-old in my section of the plane. He was on the complete opposite side, but someone thought bringing a toy drum on for him was a good idea, and he was putting it good use. I actually didn't notice at first because i was listening to music, but when I took my headphones off I heard it. I looked over and it would have been hysterical if I wasn't afraid that violence was about to break out: almost the entire section of passengers had heads turned, glaring at the little boy, and there his mom sat, completely oblivious, thumbing through a magazine.

I told my sister about it later (she's a mom) and she said it didn't surprise her at all - the mom was so used to constant noise at home, she prolly didn't even notice. She also said it was rude of the mom NOT to realize they weren't at home.

sucking & frequent swallowing usually does the trick.
a breast/bottle/pacifier/hard candy at take off & landing help a ton.

That's how I coped with my kids. My kids never screamed, but they were still experiencing pain. A friend of mine is working on an invention that will hopefully make this a problem of the past.
