The fluid in the video clip does not look like semen. It is fairly thin, and does not cling the way semen does. I have met a few individuals that produce astonishing amounts of semen, but that does not mean this clip is real.
There is a simple way to make a gusher cum scene, just like the one in the clip, if you don't mind the risk of a serious bladder infection. By inserting a catheter in the penis, it is possible to fill the bladder with fluid. (There are people who do this as a fetish sort of thing, and there are also medical irrigation procedures that do it for good reasons. The necessary catheter is commonly available, as men with various disorders must insert one to urinate.) You can then spurt the fluid out in the same way as when you urinate. If the fluid happens to be milky white, it will look like a massive cum load. Personally, I would never fill my bladder with milk or whatever, but people have done far worse things in the name of making porn.
Regarding the large scrotum in the film clip, remember that the testis only produce the sperm, but do not store the seminal fluid, so the size of the testis or scrotum has nothing to do with ejaculation capacity. In the case of the film clip, the scrotum is enlarged, but the testes are not visible in the large, round, sack. The scrotum on this individual looks exactly like it has been inflated with saline, which is a rather common fetish. (Similar results can be achieved by prolonged vacuum pumping of the scrotum, which causes it to fill with interstitial fluid drawn from the tissues, but in that case the penis usually becomes "pumped," too.)
