As a (straight) practicing doctor in an emergency department, I thought I'd add my 2 cents.
The context of an examination room is VERY clinical. I consider myself a pretty human doctor, taking the time to talk with patients and put them at ease, but even then guys always get nervous and self-conscious when they have to get the trousers down, gays or straight. I've never witnessed an erection in this setting. Genital exams or rectal examinations are done in a calm, reassuring voice, and the question of size, aesthetics, odours or anything else of that nature is never remotely raised. When people maje self-conscious remarks (and those are fairly common, usually related to hygiene), I always reassure them and I make a point out of controlling my facial expressions to keep a neutral expression. I usually succeed, even in the face of disgusting venereal diseases, farts, leaking diarrhea and the like. We really ARE desensitized, we just see so much of it, and so much worse...
