That looks like a poorly-done circumcision with a skin adhesion on the bottom. If there's hypospadias, it's very mild. It appears the downward shift of the urethra tip is just the skin pulling on the glans.

If you care to have a circumcision, it might look more like a normal circumsized penis. However, I was cut when I was a baby and the docs are not perfect by anymeans. This was before the bell device that they use now. I was left with skin flaps, bridges, and a generally scared up mess. However, I didn't have to face the discomfort of too tight of a foreskin which was the problem all along. I wish I still had a full one but some foreskin, but some is better than none, or too tight. I've seen better and worse cocks than yours. But that will always be the case for anyone. Be glad you have one at all. I have buddies who lost theirs due to war injuries. Nearly lost mine in DS1. You got it!! It was Hypospadias and it got corrected 2 times at 4 and 9 years. The hole was located about 1/2 inch below the glans at birth. By use of my 4skin they relocated the peehole towards the head. Thanks God everything works fine now.... I even made some kids without any problems
