Yes! It’s quite an experience. I live rather local to SF but went for my first time this last Folsom since I had nothing else to do and friends said we should just go and day drink.
The people who are brave walk completely naked, the ones who are even braver, literally suck dick out on the sidewalks. Multiple booths for different gay brands, food tents, and then depending on where you come from, toward the end of the block, the people crowd up and there’s like this cat walk where hired go-go dancers dance. I’d say 45% percent of the people just wear leather harnesses with maybe jeans or just their underwear or chaps with a leather vest, but a lot of people just go to drink and take in the leather culture. It’s fun, but if you plan to make a trip to SF, I recommend pride weekend. Folsom is only one day, but us who live in the Bay Area/ SF party all three days of pride weekend. It’s truly a great experience of love and the city is so lively with a lot of out of towners. Every square foot of Dolores Park practically is covered on the Saturday before the parade, and Castro/ city hall is just as lively as New Year’s Eve in Times Square on Pride Parade Sunday

Is it 'brave' to walk around naked or suck dick in public?

Uhm, yes.Imagine someone you know walking by? It’s San Francisco, the amount of times I’ve seen someone I haven’t seen in forever is crazy. Saw a friend in a jockstrap and he was shy... so yea? Unless you don’t mind sucking dick in front of your coworker then yes. It’s also a figure of speech sweetie

Over the last 10 years, as San Francisco has become more gentrified, less affordable, and less non-conformist friendly, the character of Folsom, and its little brother, Dore Alley (aka Up Your Alley) has changed dramatically. Following the current crisis, I'm not sure what will happen, and it may be a long time before we see the next Folsom Fair-- I'm not sure that by September civic authorities would authorize a crowd of 400,000 packed into the streets.

The Folsom street fair itself is very crowded, which is one of the reasons I prefer the smaller Dore Alley fair. (I've being going to both consistently for 20 years now.) Dore tends to be more gay male oriented, while Folsom is pan sexual, has a bigger vendor area, and has more people who just came to gawk, rather than participate. In the "old days," both events were much more expressive. I mean this not just in the sexual sense. People would dress in outrageous costumes and put on a sort of street theater. While this still happens, it is not so much as before, probably because the crowding has become too intense. The sexual expression is far less. It used to be the porn studios put on live sex shows in their vendor booths, and the crowd sort of did the same, but this is no more. (There are some S&M and bondage demos, but not sex.) People in the crowd will still do whatever, but it doesn't seem to have the same vibe.

But the street fair is not the only event of the Dore/Folsom weekend. In fact, I'm usually already exhausted by then. There are the various parties and other events. At one time, both fairs had huge sex parties as officially sanctioned events, with thousands in attendance. The Folsom Magnitude party was a legendary dance with an adjoining dungeon space. Then the dungeon space vanished for a few years now, and last year there wasn't even a Magnitude party at all. A similar situation happened for Dore, where the official play events are no more. There are still big club events, just no official play parties, although there are smaller such unofficial events.

Don't get me wrong, I still go to both events, and do really enjoy myself, but it's just not like the old days. But I try to do my part, rather than being a gawker. Despite the cost, I get a hotel room right in the fair space, do my hourly fetish costume changes, and entertain as best I can. (Some of the things I've done for the tourists...But I did learn never to get my hard cock bull-whipped again!) Many of my gallery pics are from these events.

All the people saying it’s crowded are correct, when it gets to the peak it gets crowded but most densely in the dancing area/where the music is playing. But overall I ran into a old coworker, managed to break off from my group multiple times to get food, more beer or use the restroom and still manage to find them. Maybe social distancing has shattered the ability for people to go to a famous festival with crowds of people around, but if you can handle a musical festival or leaving a sporting event/concert with crowds of people. You’ll do well.

No, I'm actually very shy and meek, but still do it.

There is another thread on here, called "Bravery post - full frontal with face." I replied that I didn't think it was very brave at all, and showed myself at Folsom. It's just something I became comfortable with in environments where people expected to see such, and attended the event for that reason (but not in situations where it would be shocking, offensive, or unwelcome):
