Yes, significant smell there would turn most people right off, even if they do it regularly.

But people tend to wash. And sometimes they douche out their anus/ rectum, if they think their ass might be licked. Especially in porn.

It is true that there is moderate risk of bacterial infection, and you can also pick up virals like hepatitis that way. But if you have defecated earlier that day, and are passing it normally, it's usually not a problem.
