The average PE procedure done well is definitely not "30-minutes" I am sad to say. I wish that I could say that these all went well. I have seen the good and the bad. A "good" job varies in time by the anatomy of the subject at hand.
In order to do a lengthening procedure, one has to enter the incision or incisions depending on the MD and technique. Then the cutting and sometimes repositioning begins. One has to sever the suspensory ligament and in some cases reposition it creating an anchor point. Then one has to do the same with the fundiform ligament. The most difficult part of this procedure is the cleaning out and remodeling of what is called the "penile web". This one is more time consuming because it is more variable. In an actual surgical procedure you are looking depending on the structure of the individual at between one and two hours if it is done well. This is not a quick snip-snip.
If one wants girth enhancement again techniques are variable from MD to MD. Again I do not approve of all of them. This requires multiple incisions on the penis and the insertion of the material of choice which may be a dermal graft, Alloderm or other new product to accomplish the deed. This is again in either a single or a dual augmentation procedure done under general anesthetic and takes time. A dual augmentation procedure which deals with both length and girth enhancement at the same time can take up to 4 hours on the table.
Many years ago, the FDA outlawed the injection of "silicone" into a penis inside United States borders. Silicone is being used in Mexico and there are several similar products being used in South America. The products being used in South America where cosmetic surgery including PE is available under the government operated medical programs appears to be somewhat better, and the MD's using it down there do not seem to have the problems of the people in Mexico at creating grotesque disasters.
The product you are discussing is only as I understand it approved by the FDA for facial reconstruction. There are in fact multiple products used in facial procedures which are not legal under FDA guidelines for injection anywhere other than the face and any MD using these products in ways not approved is risking his career and his medical license under federal and some state guidelines. There are many who take these risks, but, most of them as problems happen lose their licenses when things go wrong.
The main problem with injectable materials is an immune response called "encapsulation". Some parts of the body are very prone to this and others are not. The female breast and the male penis both have high percentages of problems with this phenomena. On those who will have the problem, it can start anywhere from one to five years after the procedure was completed.
To treat this one, long incisions must be made the entire length of the penis. The effect and procedure is not unlike the manner in which we peel a banana. I personally helped two men through this and then the microsurgery necessary to minimize the scarring as a "revision" after the first stage healing had taken place. The Surgeon doing the removal must then peel back the tissues and has to very carefully remove not only the encapsulated material, but he has to remove the tough fibrous tissue which has encapsulated it. This is time consuming and difficult. Some of these "removals" have taken multiple surgical procedures to accomplish. Again this is NOT a walk in the park and to the best of my knowledge there is only one remaining Surgeon with the skill to do these really well. I attended the retirement party for the second one at the end of last month.
It can cost you $1,000 to inject it and it can cost you $30,000 or more to get it fixed afterward.
I handled another case of a fellow who wrote to the estate of the late Gary Griffin. This man was from Nashville and had tons of money from the Country Music business. He spent $10,000 with a surgeon in Florida trying to turn 5 inches into 8. He then spent $50,000 trying to get the original 5 back. He was very lucky in that he had two very good surgeons and in the end many surgeries later he ended up with 6 inches and was elated.
There are all kinds of false promises out there on PE from surgeons. Most of them are no more true than the ads for herbal garbage we are all bombarded with.
There are new techniques under development. I am aware and in the pipeline for some new stuff being worked on which is totally new. At present the total number of men having experienced these new techniques is just over one-dozen. Though nothing like in the past, so far, there have already been a few growing pains with these new techniques and there have been two patients which had some problems. The only thing that these new techniques do right now is lengthening. The same MD's are working on new techniques for girth enhancement, but, these are still completely experimental and have not left the initial R and D stages as yet.
There are many forums which have gross exaggerations and other misinformation contained within their text. I operate a yahoo group dealing with the aftermath of what happens when these surgeries go wrong. In that group, I have multiple members who are in the employ of these surgeons, and, because many of these men are paid by their employers on commission they have multiple group identities. I suspect that one fellow may have as many as seven identities. The game is talk up the surgeon who pays you those commission checks, and call all the other guys "butchers". If you need to, you make up another false identity and you say: "Dr. Jones is just great I was in and out and Dr. Jones helped me gain 3 inches." or "Dr. Smith is a butcher he abandoned me and he would not see me after I had a problem, and he refused to help me." This is another way to attract business away from a competitor.
The biggest guideline is that to this day the FDA has never given it's stamp of approval to any current form of PE surgery because of the track record and percentage of problems per number of surgeries completed. These surgeries have existed now in the United States for more than 20 years. Dr. Ricardo Samitier introduced these surgeries into Florida more than two decades ago. Dr Samitier by the way is now in prison because he got "sloppy" and a simple PE surgery resulted in death. Samitier in the beginning actually had good results. He charged about $3,500 for lengthening and before greed took over, he did a good job. When it became this incredible "cash cow" he started working more and more hours, and becoming more and more sloppy in sterile techniques and other things which created problems. There are incredible stories of problems towards the end before the death.
