I do know what you're talking about when you say you can 'just decide' to have an orgasm. Sometimes when I'm masturbating, usually when I have cum once or twice already but haven't felt quite sated, I find myself with no time or inclination to continue - I hate stopping without finishing but I find I can tense completely and have if not an orgasm certainly a climax of sorts. Maybe others will disagree with me on this but the sensation I get when I do that is what I consider a 'lesser orgasm'. It's not something I've ever tried during sex with someone else but I don't see why it would be any different.

The sensation I get from it is good and satisfying but it is nowhere near the release and pleasure I get from a full on orgasm. What's surprising, of the three orgasms the clitoral is the easiest to attain and it happens to be the largest organ when nerve count is considered.
