If it's a three class cabin and the flight is over 4 hours I book business... and sometimes upgrade to first. I have a Hawaiian Airlines credit card and get miles and upgrades from that. I've flown over a million miles with United/Continental so I'm fairly ensconced in their program.
For a twelve hour flight from NY to Honolulu it's worth it to sit in a nice seat and eat something decent. A 45 minute flight from Honolulu to Hilo I can deal with coach.
Favorite first class flights have been on...
Pan Am First/Clipper Class
Western Airlines
Cathay Pacific
Qantas
JAL
And Continrntal from NY to Tel Aviv was pretty swell too.
Swiss Air (Could not find a clip)
