Used to happen to me a lot and my doctor said it was because he was too big, then she laughed, and said, no, just get KY liquid. Foreplay and lube. The stretching exercising are easier when you are pregnant because your body is getting ready for the big event, but, it is really important during foreplay to gently strech out in order to prevent this from happening again. I never even needed an episiotomy while giving birth, just minor tears and no stiches, so that tells you something there, because the whole time I was pushing my doctor was gently stretching me.
