Hemoroids? How is it done?

Just a simple, but weird inquiry....just our of curiousity;
How does a gay couple handle a "bottom" having a bad case of hemoroids?
I just developed hemoroids and have difficulty even touching the area to clean it.....while showering this question popped into my mind.
