It depends on the lycra. Compression shorts are made specifically to hold your equipment up and in place so as not to cause friction or too much movement. That dude doesn't look like he's wearing anything underneath.

Ironman triathlete here. Bulges are a great benefit to doing triathlons! You do NOT wear anything underneath a tri suit because that suit is what you swim in, what you bike in and what you run in for the whole race. I have often said that wearing tri shorts is as close to naked as a guy can be while being clothed. They don't have the padding that normal bike shorts have and they way they are cut pretty much leaves no room for your cock and balls since you can't really let them hang there. Your junk is very supported in in them and they don't move really at all. If anything a few times in a race a guy might move his around to make sure its still there! LOL! There is more than one pic of me racing where you can pretty much see all the I got!
