Always positioned it straight up when I get a hardon, it pushes way too hard against my pants to keep it down. Then I just tuck it behind my belt and avoid overhead stretching.
Has to go up. When I get hard it points a little above straight out, so if I get hard in public and it is down, then it would stick straight out from my pants.
