I can tell. And I can give you some pretty reliable hints. But first a little background:
When my wife and I met (back in the late 80's) and were becoming attracted to one another, we talked a lot about honesty before we had sex. I said I'd prefer a sex partner not to pretend to have orgasms to try to please me. I said that if she didn't come during intercourse that would be fine as we could always find a way to get her off after me if I came first. This was me speaking as someone with premature ejaculation issues. When we did have sex it was very good. She didn't come, although I could tell that she was trying to. I lasted better with her than I had with a few of my previous partners, and I realised later that it was because I felt more 'secure' with her. Eventually, she managed to relax and just feel it rather than chasing the phantom orgasm and one day when I was fucking her real slow and keeping it buried deep (it was a way I'd found of staying in control), she started to come, and it built and built and she stayed relaxed, then we exploded together - her orgasm triggering mine. There is NO MISTAKING the real thing when it happens. The texture of the vagina changes as orgasm starts and you do feel it open as it 'tents', plus there's a lot of extra wetness and (in my wife's case) her hips seem to almost dislocate in an attempt to get me in even deeper. Straight after the contractions of her orgasm subside I always noticed that my wife's areaolas would be scrunched and her nipples would be standing up erect for a few minutes even in very warm weather. Sadly, that particular show is a thing of the past as she has now lost both breasts to cancer, but I trust myself to know when she's having an orgasm, and more importantly, I trust her to tell me the truth about, well, anything. That's why I married her.
