Okay, am I the only guy that struggles with this? I get these kinda painful ingrown hairs in the clefts of my groin (along the sides) that come and go, but have increased a lot recently. I don't wear too tight of clothing and I wash regularly. Any thoughts?

It wouldn’t hurt to see a dermatologist. As a retired doctor you may see this as a theme in many of my posts. There are many reasons for ingrown hairs and of course it’s impossible to diagnose from a distance.
Are you shaving the area? That would be the biggest culprit but you don’t say. You can buy an exfoliating glove from any box store and lightly scrub the area. Choose a glove on the softer end of the spectrum.
You might try any of the cleansing washes that are on the market. I used to recommend pHisoderm but there are many other options. Use this only in the affected area and be aware of creating dry skin with prolonged overuse. Hibiclens is a great antiseptic that is readily available and typically used prior to surgery before the patient comes into the hospital. It’s not something you would use daily.
The real question you’ve got to answer is why. Is this a chronic problem? The fold of skin where the leg meets the groin is a common area for infection and rashes from birth to death. I won’t get into the weeds of all the possibilities and silly medical names that you’ll never remember. That’s why I still recommend seeing a dermatologist or your general family doctor. It’s truly the most expedient way to a solution.
