I'm guessing he was a crystal user from the scenes you describe. Crystal relaxes the hole hence the ability to take huge objects. It takes a while for sexual behavior to catch up to being sober. No wonder he ended up with stitches. It's not a surprise that sex takes more gay men out of recovery than anything else. When I got clean (10 years ago) I didn't want sex at all.
