Having a varicocele can defintely lead to low hangers. The purpose of the scrotum is to maintain lower temperature in the testicles which is necessary for the development of healthy sperm. When the temperature goes up, either from increased blood flow or high ambient temperature, the cremaster muscle in the abdomen and the dartos fascia (nutsack crinkly skin) relax, extending the boys further and making them much more pendulous. Thus, low hangers.
That varicocele is increasing blood flow to the wiggler factories, which results in a lower hang. In many cases, however, the hang is still insufficent to air them out, especially if you're a briefs kinda guy. The best solution is to get that bad boy removed, as I did many years ago. It seems to have worked out ok, as I've sired two foals in the years hence and not had any problems since. My surgery schedlue was accelerated, as I was having a hernia repair done and that patching would make it more difficult to access the jewels. PM me if you want the gory details on my experience with the procedure, but Wikipedia has a pretty good varicocelectomy bit and should help.
