I have never had a "body rub" with a happy ending. However, I do understand how these things get done.

Proper etiquette says you allow the person performing the service to touch you as s/he sees fit. However you should not touch that person unless invited.

If you go to a massage parlor, a tip would be customary. If you have someone come over from a service, a tip would still be customary. These workers split the hourly fee with "the house", and not always 50/50.

If you have an independent coming over, a tip would be optional. If the ending's extremely happy, I'd say tip generously.
