I'm shocked some people are taken aback by the idea of people they find hot using steroids, getting surgical enhancements etc.
These people don't just do this for validation lol, this puts food on the table and a roof over their head. They of course are putting work into their "craft", they don't just wake up and put the camera in front of themselves.

But it isn't wrong to express the difference on the looks of those we like. We do not necessarily mean it in a shady or derogatory notion.

When a person comments on how someone else's superficial looks have changed and they immediately attribute it to surgery, injections or steroids (especially without any proof), they're not being complementary.
