measuring with fabric ruler?

When using a fabric tape measurer/ruler, in regards to length, is the standard to hold it out over the head? or let the ruler drape down the head and measure at the head's tip?

though the illustration on the page is wrong and if i measure with the ruler at the side of my penis rather than on the penis i measure an increase by half an inch.

using a stiff ruler is way better for accuracy... of course a cloth tape would follow the head contour thus adding extra tenths of inch...
