I think the original image was just some guy stood in a pub, penis neatly tucked away and not on show. The photoshoppery was the addition of the penis, which I believe may belong to Matt Hughes (although that's a guess).

It's definitely photoshopped, you can tell by looking at the sweater hem. Also, notice how he's holding a beer in his (supposedly assumed) dominant hand. It's unrealistically difficult to hold something filled with liquid in your dom hand and then try to undo your pants with just your other hand. Even just holding a cellphone in one hand and attempting to get your junk out with the other is hard to do when at a urinal.
