"Followers" = "Viewers" = "Fans" etc. That's how media has existed and evolved for nearly 100 years.

Can you imagine waking up every morning and dropping a "Thanks" or "Nice Post" to everyone you follow on social media? You'd have be insane..

More likely to = bots.

I can imagine more than 0.01% of 14 million people doing it because they do it on plenty of other pages. The 0.01% and lower engagement is on all his posts, not just a few. Other pages with a fraction of the followers have significantly higher overall engagement.

Engagement isn't just comments. Most engagement on social media is likes. If someone has 14 million followers on ig but gets like 1000 likes in a post, that's almost 0 engagement.
But it is true that companies often times don't look at that. They'll sponsor someone who has millions of followers because they think that'll convert to views, when it never does

yeah. that means all those people are inactive followers. they still follow him because they don't bother cleaning out their following, but they don't bother with engaging with any of the posts, they just scroll past them. obviously there's also gonna be accounts of people who died, and accounts that aren't used anymore(people in fandoms often times have multiple accounts in the same social media platform, so sometimes the followers are just the same person twice).
but that just means the number of followers is inaccurate. there isn't an inactive followers purge. if there was, it's more realistically reflect the actual number of followers
