It does have the traditional side effects of nitroglycerin (also used to treat chest pain and anal fissures): lightheadedness, dizzyness, flushing, etc... not good for people with heart problems. These side effects might be reduced because of the smaller dosage for erectile dysfunction.

From the description, it sounds like the closest thing to a working penis enlargement cream, though it wouldn't have permanent effects.

An alternative to nitroglycerin is Vitaros by Apricus. It's an alprostadil cream that has been approved in Canada and Europe, but was rejected by the FDA for safety reasons: Apricus mulls options; to sell U.S. rights to Vitaros | Reuters

If I were writing fiction, I'd speculate that the Viagra / Cialis mafia shut them down, but there might have been more mundane reasons for the failed application as well. Apricus' stock price dropped dramatically after the announcement and it sounds unlikely that they'll recoup their investment in the short run.
