It's obviously not real. A common "trick" is to inject the bladder (through the urethra with a catheter or syringe) with a milk/corn starch solution and piss it out in spurts to make it look like an ejaculation. The guy's not even erect, as notice the flaccid pull in the closing shots. His ball sack is obviously also injected with either a saline or other type of solution.

Like FLBobcat said, It's from injections. Nobody's balls would look like that normally unless they have a serious medical condition or Saline/enlargement hobby.
It can't be that pleasurable because he isn't even erect.

This video has been around for years. I think that there are a total of 3 or 4 videos of this same guy doing this. The deal is that he uses a urethral catheter and makes up a solution which is infused up into the bladder and related structures. The solution used for this can vary, but believe me the dangers for bladder and urethral infections from doing this one are monumental. AS has been said the fellow then releases and clenches the PC muscle to simulate the ropes of an ejaculation. It can look very real to an untrained eye, but this is a type of fake. There are others who have done this one and again it makes for great screen shows but it is definitely not ejaculate material coming out of that penis. Sorry to disappoint.
