Get over it, it's a doctor's job to examine the entire body, this current (hopefully short-lived) trend in some regions of some countries toward total freak outs about nudity must not get in the way of the important job of a doctor. Any doctor who let's any fear stop him from doing his job, shouldn't be a doctor. Seriously this kind of exam is important, as demonstrated below:

I had a physical a few days ago (my arm
still hurts from the tetanus shot :grumpy
and the genital/hernia/prostate portion of the exam went very much like this video, with me wearing a gown but naked underneath. (I left my underwear on until we got to this portion of the exam.)
When I had an exam a few years ago, the doctor had trouble finding anything for me to wear other than an old, threadbare gown that didn't fit well. I only wore it for a couple of minutes before he said, "Oh, just take the stupid thing off," which was fine by me. However, I was still wearing my boxers and he didn't ask me to remove them until we got to the hernia test. (At that time, I was young enough to not need a routine prostate exam.)
For previous physicals, the doctor would have me strip down to my underwear, and would have me pull them down only for the genital/hernia portion of the exam.
Although I'm not inhibited about being nude in front of a doctor, I think I would be uncomfortable being completely nude throughout the entire exam. Among other things, those exams rooms are always too cold.
