A few of things to note about the document:
(1) The bottom of the document identifies its origin as "Office of the Principal Registrar, Coast Province, Republic of Kenya."
(2) The signature at the bottom of the document is dated 17 February 1964.
(3) The place of birth is given as Mombasa.
As of 17 February 1964, Kenya was called the Dominion of Kenya. It only took on the name "Republic of Kenya" ten months later, on 12 December 1964 (source). Also as of the date of Obama's birth, Mombasa belonged to Zanzibar. It was ceded to Kenya only on 12 December 1963 (source). [Corrected.]
Some have also considered it suspicious that the registrar is identified as "E. F. Lavender," said to be the name of a brand of soap (though I have not been able to confirm this), and that the number on the document is 47,044, when Obama was 47 years old when he was sworn in as 44th president of the US. So it may be that the document was designed as a hoax on the birthers, to see whether they would be smart enough to figure out that it is a fake. I don't know where Taitz got the photo from.
It is also amusing to note that it is only a "certified copy of registration of birth," not a birth certificate!
