Outsize balls

Have often seen a guy at the pool, middle aged, fit with a reasonable sized dick and very large balls - not low hanging but a bit like a large grapefruit - then this week saw him and they seemed to be even bigger, more like a melon. Didn't look at all healthy, or comfortable. Any idea what could be causing this?
