I've done some sounding in the past and have bought a few PA plugs which are difficult to keep in, but I like the way this one clips onto a glans ring. Very clever. Yes, it's a natural reaction that plugs are pushed back out, they don't belong there after all. So that's why I bought this plug with a rubber ring attached to it, in fact it's a head ring. It should be tight so that it won't slip off when you're soft, and it will keep your plug inside. Always use plenty of lube. The plug will go in as deep as 2.5 to 3 inches, and it's pretty thick. Have a look at the pics I just made, so you'll see how it works.

And don't worry about the pressure of the ring. At least, for me it was no problem at all when having an erection, it just felt good.
Today I've been wearing it for about three hours. As you said it's better to wear loose underwear, tight underwear is very uncomfortable. I didn't use any underwear at all, that's even better! I have the same one. I've worn it up to 3 or 4 hours but for all day I prefer the piss/cum through ones with a hole. That way you don't have to take them out to pee. Also, you have to take into account the length of the plug and the smallest size you might get flaccid, whether you'll be mostly upright or sitting. They can be a little painful (especially the piss through ones) if they penetrate too deeply because of the edge of the hole.

The reason why I don't wear plugs with a pee or cum hole is that it's not easy to completely clean such a long hole. It's a source of infection! It seems like fun, cumming or pissing through the hole, but on second thoughts, no.

Wash the plug in hot soapy water, use a pipe cleaner in side the tube. Don't reuse the pipe cleaner. Then use the pressure cooker, 10 minutes at the least.

In the meantime, I get an 18mm plug into my Meatus Externus inside. That's why I now permanently used an 18mm penis plug with longitudinal bore. So I can again control pissing.
