The other people featuring in this series of videos are real patients with actual complaints, but as this is a instructional video about how to examine the testicals for testicular cancer, they probably hired a art class model.
By chosing this model, they are not suggesting that all men are as endowed as him, same as they wouldn't suggest that if he had been less endowed. We're all different so I do not see anything wrong with allowing a well endowed guy to appear in one of the videos
