random hookups happen all the time, and I get that most guys are on prep, so HIV is not a huge risk factor. But what about all the other scary diseases that you can catch from a bj or even kissing? How are guys not concerned?
 
From my experience in working in the HIV field for several years, kissing and blow jobs are not the culprits in the rise in STIs that are out there. The use of PrEP without condoms has caused the increase in STIs, especially among gay and bi men. Rates vary across the nation, but the south and larger cities nationwide where Grindr is popular have seen increases.

I'm straight and used condoms for intercourse with every single woman I've been when things became sexual.
In hookup type situations, condoms were always used and always expected, by both of us.

In one long-term relationship, I continued to use condoms every time as she was not on birth control.
Only in another long term relationship did condoms eventually become unnecessary as it was a committed strictly monogamous relationship.

There are sooo many STDs/STIs out there, play safe!

I'm actually very concerned and although I might meet someone that I find attractive and they want to have sex, I am somehow able to talk myself out of it for quite a while. I think about the other stds that I would be able to catch even if someone claims to be on PREP. When I was in high school and college I took a lot or risks with random hookups but luckily never caught anything. I always felt great regret after the act because I started fearing exposure to something and that is the worst part for me.

I think prep, although an amazing drug, has had the adverse effect of making gay men less weary of other STI’s which I think is like straight men not wearing a condom if the girl is in the pill, gay men don’t wear one if they guy is on prep. If the ‘worst’ can’t happen they aren’t that bothered.
 
Yes, many guys are deciding not to use condoms because HIV is much less scary today than it was 20 or even 10 years ago.

You are correct right to be careful. For example, HSV (herpes) is easily transmit-able and will forever stay in your body. Most people don't show any signs even when they're highly contagious. Alas, in the US they don't screen for HSV unless the patient insists (or if there are symptoms). Most people don't know that cold sores can easily cause genital herpes (via oral sex), again, even when there aren't any sores. Look up the statistics

My advice: play safe. Don't let anyone make you feel uncomfortable
