Relationships gone wrong

My Bf and I have been together for almost 3 years. But the last few months have been very trying. Sex has been few and far between. He's always tired. He doesn't like to be touched although hugging and cuddling is ok. He does not like to be touched on his balls, he giggles. Don't go near his ass or his ass crack or God forbid his hole (he's 100% top). He doesn't like rimming, he doesn't like sucking to the end. He is just cold. There is an age difference - he's 23 and I'm 52 and I'm his first relationship. But given that I can't figure it out. My neighbor says that she thinks this all means he's getting it somewhere else. He goes to visit his family at least once a month and when he's here he goes off with his friends a few nights a week. I never thought that he would cheat but all of this is not adding up.

Any advice on how to handle/address this?
