From all the amateur sleuthing done on this thread by fans the consensus seems to be "yes" it is real. Full nude shots were taken in the Body Issue, not just Gronk but others, for years nothing was leaked. Finally a few leaked of other athletes and this one of him. Also there was a locker room interview of a teammate of his where Gronk walked in the back into the shower nude and we saw his cock there too, and his cock there looks very similar to this shot here.
