The song that's brought me to tears for 20 years, the only song I tear up at every time I hear it, is "What's the Matter Here" by 10,000 Maniacs.
The section that breaks me to pieces is:
If you don't sit in your chair straight
I'll take this belt from around my waist
And don't you think that I won't use it
Answer me and take your time
What could be the awful crime
He could do at so young an age
If I'm the only witness to your madness
Offer me some words to balance
Out what I see and what I hear
Oh these cold and wrong things that you do
I suppose you do because he belongs to you
And instead of love and the feel of warmth
You've given him these cuts and sores
That don't heal with time or with age

Other really sad songs:

"Save Me" by Aimee Mann (from Magnolia)
I remember being stunned by how well that one song captured the entire sense of the movie.

"Hallelujah" by Jeff Buckley
His voice in the live version of this song could break your heart into a million jagged little pieces.

"I Will Wait For You" by Connie Francis
The longing in the song is brutal and totally relatable. And this will sound dorky, but it was used in a scene in Futurama once, and my gosh was that scene powerfully sad.

YouTube - The Brothers Four - Try To Remember

Try to remember
The kind of September
When life was slow
And oh so mellow

Try to remember
The kind of September
When grass was green
And grain was yellow

Try to remember
The kind of September
When you were a tender
And callow fellow

Try to remember
And if you remember
Then follow

Try to remember
When life was so tender
That no one wept
Except the willow

Try to remember
When life was so tender
That dreams were kept
Besides your pillow

Try to remember
When life was so tender
That love was an ember
About to billow

Try to remember
And if you remember
Then follow

Deep in December
It's nice to remember
Altho' you know
The snow will follow

Deep in December
It's nice to remember
Without a hurt
The heart is hollow

Deep in December
It's nice to remember

The fire of September
That made us mellow

Deep in December
Our hearts should remember
And follow ... follow ... follow ...

I dont know why, but whenever I hear True Colors by Cyndi Lauper, it makes me feel sad, though its an inspiring song in a way as well. That would be my sad song for this topic

I think Dr Hooks Daddy's little girl is one of the saddest songs I've herd.
but then again i think the saddest thing in this world is to outlive your children.
Those who have done it will know what i mean.

She'd snuggle up and talk to me
Each night as I'd watch TV
I'd teasingly call her chatter-box
Then one night she said "Don't be scared,
I see an angel on the stairs
Daddy can you hear the angel talk?"

One more year of lollipops, ice-cream cones and soda pop
One more year of Daddy's little girl
One more year of cracker jacks, bubble gum and sugar smacks
One more year of Daddy's little girl

I took her in my arms upstairs
And as she said her night-time prayers
I felt a fever burning in her head
She gave thanks for Mum and me
And said, "I heard my angel speak
Daddy this is what my angel said

One more year of lollipops, ice-cream cones and soda pop
One more year of Daddy's little girl
One more year of cracker jacks, bubble gum and sugar smacks
One more year of Daddy's little girl

Her faith grew strong, her body weak
And soon my frightened eyes could see
There was nothing on Earth that I could do
And one year later, to the day
In her sleep, she slipped away
And I knew what she'd said and heard was true

One more year of lollipops, ice-cream cones and soda pop
One more year of Daddy's little girl
One more year of cracker jacks, bubble gum and sugar smacks
One more year of Daddy's little girl

One more year of Daddy's little girl
