Serious questions around TRT, testicular atrophy and full package pumping

After several years of dealing with low testosterone levels, and researching/trying multiple treatments to increase my test numbers naturally (with no real success), I’m finally taking the steps to begin Testosterone Replacement Therapy with my current primary care physician. I’ve shared with my doctor my concerns around side effects of testicular atrophy from being on test, and possible methods of reducing testicular shrinkage while I’m my TRT cycle. My doctor didn’t seem to be aware of any methods to prevent this common side effect, but after roaming various forums around TRT and steroid discussions (like this one), I know others have shared various methods to reduce testicular atrophy while on the juice.


One of the methods I noticed rarely explored in keeping the family jewels heavy while on the gear, is the use of ball/penis vacuum pumping (aka full package pumping). I know of several bodybuilders, powerlifters, weightlifters, and just regular gym heads trying to get bigger, who full package pump while on gear to maintain (or further enlarge) their testicular size. Yet, I rarely seen a deeper discussion around the experiences of pumping muscle with juice, and pumping your package with air pressure, specifically in the arena of preventing testicular atrophy. Full transparency, I have already briefly explored full package myself before getting on TRT, which had overall good results with personally.


So for the more experienced gear heads here, who have tried full package pumping while on TRT, roids, or other androgen based cycles, can you share your experiences in this method possibly reducing testicular atrophy?

I explored pumping during covid, enjoyed two months of regular sessions around 30 mins twice a day- results were a heavier cock and noticeable vascular texture, but my hardons were not as ironhard. Pumping does not increase the size of your testicles but it does make your ball sack appear bigger by drawing fluid into the skin.

The testicles temporarily get smaller because they're simply not used anymore to produce testosterone due to foreign testosterone entering the body. The good news is that the size will return once you have jumpstarted your own testosterone production.

This is usually done with after cycle therapy which is usually clomid or tamoxifen a fertility enhancing medicine for women that increases your estrogen levels to which the testicles respond by producing more testosterone to counter the estrogen.

Alternatively you can use ginger during your testosterone cycle to keep your testicle size as this research article below explains.

Results: The aqueous extract of Z. officinale significantly increased in the relative weight of the testis, the serum testosterone level, testicular cholesterol level and epididymal a-glucosidase activity. The aqueous extract of P. brazzeana significantly increased the weights of the testis, seminal vesicles and prostate. It also significantly increased the serum and testicular testosterone level.

Additionally you can also apply an ice pack 2x a day for 20 min to your testicles.

I’ve been on TRT for several years now, doing injections. I guess my testicles have shrunk some but it’s not a big deal to me. Of course I would like them to be a little bit plumper. The main benefit from the TRT is it makes me so horny, especially when I up the dosage from time to time. I feel like a 16 year old again!

Being used lot of different androgenic compounds for decade or more.
Lot of literature read and have a strong background on pharmacology .

Normal TRT , acceptable range not super high , won't shut down all the final production in testicles, even when lh or fsh is almost zero..That's right the word is almost .
So science really haven't figured this part out yet, but there seems to be still some cells stimulation, just not enough to produce testosterone.. or sperm....

Theory is this, you use enough testosterone that you're lh fsh zero point something, sperm count zero...
But if you add enormous amount of more androgens in they will get even smaller, why? you were already shut down.
And when you drop almost all the extra androgens and go back to trt, still zero, they gain little bit size back up...why?
There seems to be some stimulation in cell level still , but not enough to "finish anything" hard to explain. They work but don't produce would be easier to explain...Plain trt not extra hard androgens won't "shut down" them...not hard atleast..
All this takes time being said, it's not a over night change, but in (weeks) months usually.

All this being said, i would be extremely interested, if non trt person, would pumping testicles change something in testosterone levels/sperm count... Or how the leydig or sertoli cells change..
If there won't be any change at all in those 4 things, it's all temporary cosmetic fluff...

But i'm very interested in this, this would change a lot in medical science world.. If someone who doesn't ball pump, do bloodwork , and start doing a regimen..
