Sex Chart Documentation - HELP!

So I wanted to start a little black book, so to speak. I have this little book that I want to start documenting all of my sex eskipades in. (spelling?)

I was curious as to what suggestions you guys would all have that I should include in my book. I'm thinking about only having 5 or 6 columns, and whenever I have a new incident I'll go in and add a new line to the page, so eventually... The book will have pages full of line after line, ect.

I was thinking about having the columns go something along the lines of...

Name - Date - My Age/Her Age - How long the sex lasted - Rating

Any suggestions of what else to add?

My book has the following; name, phone no., age, estimated stats, and then ratings on looks & sex. I also make odd notes about kinks and habits if anything really turns me on or puts me off.

I don't keep track of each sexual act, I just keep track of how many people I've had sex with, in a file on Excel that I call the Partner Chart.

Here are the columns I have:
1)First the number they are with regard to just messing around
2)The total number of people that I have had anal sex with number
3) Date of the first encounter
4) Date of the last encounter or a note that it was just a 1 time thing
5) Name
6) the persons age
7) where the person is from or the location of the sex
8) Phone number

The next columns are about what we did together:
1) Talked
2) Cuddled
3) Kissed
4) JO
5) Touched
6) I sucked them
7) they sucked me
8) I fingered them
9) they fingered me
10) I fucked them (in this column I put a note whether we used condoms or not)
11) They fucked me

Last I just put some general info columns (though these don't always get filled out)
1) Notes
2) Address
3) Screen names
4) where we met etc

Two ratings, one for looks and the second for performance. If you are anal, you might rate each area, such as oral, vaginal, anal if you are able do it.
