I'm not in that age bracket, but I was curious. The questionnaire certainly asks a LOT of personally-identifiable information. I didn't fill any of it in, so I didn't get past the first page, but I sure would not send all that info to some person I didn't even know!

I went through the entire questionnaire (did not fill in any part) and can only conclude that this so called questionnaire looks more like an online form for a gay personal advert.

It asks for your personal details ranging from

Personal apearance.
How often you frequent gay/straight bars.
Type of sex you like and what type of guy you're into (top/bottom).
Medical history (HIV +/-)
The list goes on.

Why else would it ask you at the end of the questionnaire if you would like to submit pics?

Yes, 16. It's a British site, as shown by the .org.uk extension, and the British age of consent for both gay and straight is 16.
