Brokli, this sounds horrible! What you are going through does not sound normal and I think you should start thinking about the bigger picture. How is your overall health? At twenty-four it should be excellent.
I had an ongoing problem with recurrent bronchitis. Antibiotics would clear it up only to return in two or three weeks. My very nice but conventional internist referred me out to pulmonologists who diagnosed me with asthma and then COPD. I got bronchitis AGAIN while in Mexico and went to a doctor at a corner pharmacy. He talked with me for about half an hour, examined me, told me I had some kind of underlying infection that had not been adequately treated, and prescribed a gram of Rocephin daily for five days. He was clear hat the treatment wouldn't cure the underlying cause but would get me back to the U.S. A couple of years later was diagnosed with chronic Lyme disease. That was fourteen years ago. I started getting treated with antibiotics and have only had bronchitis twice since. No asthma, no COPD. After I was treated for a while, I came to realize that I had been sick all over for years and the bronchitis was the most pressing symptom.
You are falling into the cracks of the medical system which mostly sees patients in fifteen minute increments, makes to most likely diagnosis given the nature of the primary complaint, and prescribes accordingly. This is not going to be easy and the medical establishment is not going to wake up and realized you have been overlooked and spring into action on your behalf. You are going to need to find a physician who is less bound by convention and convenience and who will actually take time to figure out what is going on and what to do about it.
If you are having multiple undiagnosable health problems Lyme disease is probably worth considering. Quite a few young guys with it have problems with prostatitis and low testosterone. This is particularly true if you have ever had a known tick attachment and/or a bullseye rash. Regardless, you are going to need to start doing research, thinking analytically, and viewing physicians skeptically. I know. It sucks. If I can be of help to you feel free to message me.

I had a problem with epididymitis several years and it was quite painful. It was an infection that lasted for many months. The urologist finally put me on a sex week course of antibiotics that finally cleared up the infection. This may not work for you, but if they haven't tried this already it may be worth a try.

Quercetin has also worked for me.

get Epididymitis out of nowhere. It has been happening for years. I always had to go to the doctor as Antibotics as it was the only thing that would make it better after a few days.

I read about a vitamin a year ago called Quercetin, that's suppose to help with Epididmyitis and I started taking it, as I figured what I have to lose. Well holy Sh*t does it work, much better than the antibotic.

Once I start feeling any discomfort I take 2 pills a day until it goes away. It works. Just an FYI

Epididymitis is a common problem for a lot of men. Anal sex with no condom can cause it from colon bacteria that gets into the urethra. It wasn't meant to go there guys. No moral judgement here just common sense biology. In most cases it is caused by urine that backs up there. Have you been checked for prostate problems? That is the number one problem that causes epididymitis see a urologist. With some removal of the testicle that is involved is the only permanent remedy. That may not be on your plan chart but when it gets bad enough sometimes it is the only solution. Two specialty groups on this subject are

Yahoo! Groups and Yahoo! Groups

If anyone feels the need.

I've been going through with this myself for years and its so very easy for doctors to give up...I had just hoped it was Australian doctors myself given the reputation here for out overworked medical system.

Right now the pain is at the worst it's been.

All I can say is keep hanging in there bud.

It sucks I know.

I did a whole round of specialists myself back in 2014 (the issues originally developed in 2011) after I pushed for them to do more than regular stuff and went full gamut with different specialists and basically they gave up with it when all those specialist's regular treatments failed (though i still wonder if it is prostate related myself and low testosterone-given my last T score was on the lower side of normal).

Problem I find is the doctors really have had enough of it - I have found if regular by-the-book treatment stuff doesn't work they frankly don't want to know you.

I spent 12 months on 2 different types of rarely administrated super strong anti-bios here back in 2014 that at the end of the day didn't resolve it at all, but there was never any sign of infection on any of the standard tests or in me myself, so frankly I personally wasn't surprised the anti-bios didn't do anything for me.

A few things looking back at the forums posts i've not tried were simple things like epsom salt baths. Surprised that they help some people actually. Personally I've been on Quercetin for ages for something unrelated and it really does help with inflammation (works fucking miracles on Mosquito bites btw!). But I find it does nothing for my testicular pain.

My urologist doesn't recommend testicular removal given it will most likely lead to 'as strong and constant' phantom pain apparently, and given it isn't something like Testicular cancer, he never advises guys to do that regardless that the pain is so excruciating.

I can related to what you said about tingling down your legs, the aching testicles (both mine ache though the left is the worst), the sore nuts after orgasms. I get that too. Frankly it hurts to just dry them off after a shower no matter how gently I dab. Even a hairdryer makes them hurt :( Walking is a nightmare and feels like i'm getting kicked in the nuts with every step. I sometimes just don't bother cumming when its soooo sore. It is as overwhelming for me too. As you, I have tried to blow more loads and I just get more sore. If I refrain from having many orgasms in a given week then it seems to become irritated but in a more achy way too. I can't remember the last time I came more than once or twice a week now. I am 30 myself and it will be year 5 this year for me having to deal with this :(

My loads themselves have been a bit on the smaller side lately tbh. Even my erection has been suffering a bit. The doctors aren't interested there either- I get the whole 'it's your chronic testicle pain, what can you do', from them which is annoying to say the least given I hope there isn't something else going on (like cardiovascular) and this isn't another symptom they should be taking notice of. Not one has mentioned chemical epidydmitis caused by a backflow of urine, so that's something I might broach next time myself, but I do expect to get fobbed off from the specialists if i'm honest.

I wish I had some suggestions for you mate but frankly I don't know what to say myself.

All I can do is offer commiserations mate and know that you're not alone and I feel for you.

Zee.

I agree with the earlier comments about checking out your overall health. I suffered a few rounds of this myself and got referred to a urologist. This was all about 6 years ago. It was around that time that I was also have other issues, like skin rashes and mood issues. I decided around that time to clean up my diet and take care of myself. All the sugar, carbs, and junk I was putting into my body seemed to be catching up to me. I was a slim/fit 170 lb guy and looked health, but was far from it. After a year or two of clean living I really turned a corner. Pills, topical treatments, antibiotics... all of these chemicals aren't solutions, but masks to problems. I am true believer in that old saying, "you are what you eat."
