$500-$1000 for a fuck is not wealthy people activities. I've blown through $400 in a night at a strip club. Gay men without kids and a large home to house those kids could easily save up for one or two rounds with someone at this price range per year.

The issue is that a lot of people with middle class salaries become parents which are a financial sinkhole. $500 an hour is annoying but not impossible for a middle class person making six figures to afford as a special treat once or twice a year.

Price of a plane ticket. Not a huge deal. Pricy but doable if you want that hole or pole bad enough
