I was confused, also … after I had completed the survey, I went back and discovered a graphic that states:
Use CI (coverage index) number from picture that most resembles your SOFT state.

The graphic recommends, if there's a difference in your hard and soft state, to use the hard one if you're uncut and soft if you're cut.
