Cam's biological parents are Elizabeth Webber and Zander Smith, while Spencer's are Nikolas Cassadine and Courtney Matthews. None of those people have any relation to each other. Cam only refers to Laura as his grandmother because her son Lucky was the only father he ever knew. He was never legally adopted by Lucky but has always been seen as a member of the Spencer family.
Yeah, they are in no way biologically-related. GH should really explore this
