Huh. I’ve always been curious about scenes like this. Obviously they use modesty pouches for actual nudity but I’ve always wondered if they’re wearing something underneath stuff like underwear just in case. here, it really doesn’t look like it

I think the same too. I've been re-watching the entire series and often it's super obvious (When you are looking) they have underwear on below the boxers.

Unless showing his ball sack is important for the scene, I guarantee he’s wearing a modesty garment (basically tiny briefs) under the boxers and that’s what we’re seeing. I’ve worked in tv production for almost 8 years. If the nudity isn’t the point of the scene, the junk is not shown. It can be different in films, especially artsy or indie ones. But that’s a general rule you can count on for tv.
